Lakeside Physical Therapy and Wellness is an Out-of-Network provider for all other insurance carriers and will provide physical therapy services on a Fee for Service / cash basis. We will provide our patients with a detailed statement that can be submitted to your insurance provider (HSA, insurance companies) to request reimbursement for out of network services.
